Cancellation & Payment Policy

Comprehensive Visits & Appointments

Comprehensive Visits and other individually priced services are charged separately from the Vital Hormone Health monthly membership.

We understand that schedules change. If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice whenever possible.

Ongoing Membership

Patients who choose to initiate an ongoing treatment plan with Vital Hormone Health enroll in our ongoing care membership at $129 per month.

Membership begins on the first day of the month following initiation of treatment or prescribing. Your membership start date is based on when treatment begins and is not dependent upon the date of your first follow-up appointment.

Initial 3-Month Commitment

Initiating an ongoing treatment plan requires an initial three-month membership commitment.

This initial treatment period allows time for appropriate follow-up, monitoring, medication management, and treatment adjustments after care has been initiated.

Patients are responsible for the monthly membership fee throughout the initial three-month period, even if they choose not to schedule or attend follow-up visits.

After the initial three months, membership continues month-to-month.

Membership Cancellation

After completing the initial three-month commitment, you may cancel your membership at any time before your next monthly billing date.

Your membership will remain active through the end of the period for which you have already paid, and cancellation will prevent the next scheduled monthly charge.

Membership fees that have already been charged are nonrefundable.

If a cancellation request is submitted during the initial three-month commitment, the cancellation will become effective after the three-month commitment has been completed.

Hormone Pellet Therapy

Patients receiving hormone pellet therapy must maintain an active Vital Hormone Health membership for a minimum of three months following each pellet insertion.

Because pellet therapy continues to deliver medication after the procedure, this period allows for appropriate monitoring, follow-up, and ongoing management of your treatment.

Each pellet insertion begins a new three-month Pellet Therapy Management Period.

If you request cancellation during an active Pellet Therapy Management Period, your membership will remain active through the completion of that three-month period.

Recurring Payments

By enrolling in the Vital Hormone Health membership, you authorize Vital Hormone Health to securely maintain your payment method on file and automatically charge the applicable $129 monthly membership fee.

If a payment cannot be processed, you are responsible for providing an updated payment method and resolving any outstanding balance.

Ending Your Membership

When your membership ends, Vital Hormone Health will no longer provide routine ongoing medical management, including follow-up care, prescription management, medication refills, treatment adjustments, or ongoing clinical messaging, except as necessary for an appropriate transition of care or as otherwise required by law.

If you wish to continue treatment after ending your membership, you will be responsible for establishing care with another qualified healthcare provider.
