• If you have had recent labs including the Basic Panel (within the last 3–6 months), please text me, (737) 437-3429, so I can send you an upload form

• If you do not have recent labs, I recommend completing them prior to your appointment so we can review everything together in detail.

You have the option to send this lab list to your primary care provider to have them ordered through your insurance before your visit.
